Transitions provides an array of treatment services for people who are troubled by life problems or who are experiencing emotional adjustment problems.

  • Psychiatric Evaluation and Consultation
  • Medication Evaluation and Medication Monitoring Services
  • Individual Therapy and Counseling
  • Couples and Family Therapy
  • Play Therapy for Children
  • Group Therapy
  • Crisis Intervention Services
  • A Continuum of Day Treatment Services
  • Monitoring of Inpatient Treatment for Children and Adolescents
WHO IS ELIGIBLE
FOR SERVICES
People of all ages who are experiencing personal, emotional, or social adjustment problems.
COST OF SERVICES Depending on the needed behavioral health care service, the following sources of funding are available:

  • Insurance
  • Illinois Medicaid
  • Medicare
  • Office of Rehabilitation Services
  • Self Payment - Sliding fees, based upon family income, are available to Adams County residents who have no other source of payment.
WHO TO CONTACT Referrals are accepted from individuals, community agencies, physicians, clergy, school personnel, or the courts. Referrals or requests for information can be made by calling the Client Services Manager at 223-0413.
