Behavioral Health Services

Transitions provides an array of treatment services for people who are troubled by life problems or who are experiencing emotional adjustment problems.  Services provided include:
bulletAssessment, Consultation and Treatment Recommendation
bulletIndividual Therapy and Counseling
bulletPsychiatric Evaluation and Consultation
bulletMedication Evaluation and Monitoring
bulletPlay Therapy for Children
bulletCrisis Intervention
bulletCouples and Family Therapy

People of all ages are eligible for services.  Cost of services depends on the needs of the consumer.  Services are funded through Illinois Medicaid, third party insurance, or on a sliding fee scale.

Referrals are accepted from individuals, community agencies, physicians, clergy, school personnel, or the courts.  Referrals or requests for information can be made by calling the Consumer Services Manager at (217) 223-0413.
