COMMUNITY BASED SERVICES

Community based services are available to individuals with a serious mental illness via Case Management Support Services (CMSS) or a developmental disability via Client/Family Support Services. Community based services provide people with mental disabilities the support they need to live successfully in the community.


CASE MANAGEMENT SUPPORT SERVICES (CMSS)

  • Improve Social and Emotional Functioning
  • Improve Problem Solving Abilities
SERVICES PROVIDED
  • Individual and Group Counseling
  • Independent Living Skills Training
  • Case Management and Service Linkage
  • Psychiatric Services, including Medication Evaluation and Medication Monitoring
  • Social Skills Training
  • Crisis Intervention
  • Advocacy
  • Psychoeducation & Relapse Prevention Planning
WHO IS ELIGIBLE
FOR SERVICES
Case Management Support Services are available to people who have a serious mental illness and who live in Adams County, Illinois.
COST OF SERVICES Most people served through CMSS have an Illinois Medicaid card, which pays the cost of services. For those who are not Medicaid eligible, fees for service are established on a sliding fee scale (based upon family income).

WHO IS ELIGIBLE
FOR SERVICES
Client/Family Support Services are available to children and adults with development disabilities and their families or significant others.
COST OF SERVICES Client/Family Support Services are funded by a grant from the Illinois Department of Human Services, Division of Developmental Disabilities. Services are provided to consumers and their families at no charge.
