Services for the Severely Mentally Ill
Transitions provides case management support services (CMSS) and day programming
(Visions) for adults with severe mental illness. Visions offers groups
related to independent living and social skills including budgeting, cooking,
personal hygiene, medication monitoring, and coping skills. Services provided
include:

 | Crisis Intervention |
 | Advocacy |
 | Psychoeducation and Relapse Prevention Planning |
 | Medication Evaluation and Monitoring |

Services are available to adults with a serious mental
illness and who live in Adams County, Illinois. Fees are paid by
Illinois Dept. of Public Aid or on a sliding fee scale.
